The 2021–22 Sanaman Premier League is the third season of the top division of Sanaman football. The competition is contested by 18 teams from all over Sanama. The participants were the top fifteen teams from the 2020–21 season as well as the top two finishers in the Sanaman National League 2020–21 and the playoff winner. Following the secession of Cisamarra and Highpass from Sanama, the teams from those two countries were removed from the league, and instead replaced by newly relegated teams and the highest placed teams from the National League.
