Lesbian, gay, bisexual, intersexual and transgender rights are widely respected in Meckelnburgh. Equal rights regardless of sexual orientation or gender identity protect LGBT people in Meckelnburgh under Article V of the Meckelnburgh constitution (the “Charter of Rights”). It is estimated that gay, lesbian and bisexual people represent between 6–8% of the Meckelnburgh population, while transgender people represent between 1–2%.

Due to influences from the Church of Meckelnburgh, five genders are recognized under Meckelnburghish law: male men, female men, nonbinary, male women and female women. This system formalizes the greater recognition by the Church of the gender spectrum (‘’spektro’’) spoken in chapter 5 of the Book of Kompto. Traditionally, members of the Meckelnburgh royal family identify as either male women or female men, resulting in the kingdom’s system of neutral honorifics (e.g. “Their Royal Majesty”). Members of the Church traditionally identified as nonbinary, though both systems today serve more cultural than practical functions.
